Event Request Form

Request must be made 30 days before the event date. If form is submitted less than 7 days before event, it must be approved by finance director prior to submission. Forms MUST be submitted by the designated Elder. Check request(s) submitted by Wednesday will be available on Saturday, unless otherwise noted. For Cash Advance request: a Reconciliation Form and receipts must be return within 7 days of receiving funds.
